I wrote this two months ago, when setting up the blog. Since several people have found it useful, I thought I'd give it a Sphinn.

Definitely true about the categories.  Once categories get out of hand, they can be a real nightmare.

I don't see why the full article URL ever has to be influenced by the category or categories that it is in.

That's a flaw in thinking at the design stage.

Sure the categories can list all the posts in that category, and maybe even show the first paragraph or something, but the URL for the full article doesn't have to be even vaguely related to the URL format used by the category pages.

Forum software has largely got this aspect right for many years (but ended up creating their own different and unique Duplicate Content issues by overlooking other things).

Each forum within a site has a Forum number, and each of those then has multiple pages that list the threads, usually 25 threads to a page.

Each thread has a Thread Number assigned at the time it was written. You can move a thread from forum to
forum and its number will never chnage. You can link to the thread from multiple forums
and it will only ever have one URL for that thread.

That logic seems to have been thrown away with most blog software, and I don't understand why that has happened.
