Matt Cutts explains what Google does when it discovers a hacked site: temporary removal from the index with a "hacked site" flag, contact the site owner, and then potential reinclusion within 48 hours when everything's fixed.

I've recently gone through this with Google. Even though I never got a courtesy email from them or Webmaster Central informing me that one of my sites was removed from their index due to hacked spam,  I did file a report with them informing them of the matter and almost right away my site returned.
