Bill Slawski has posted an article at SEM Clubhouse about Google's recent advances in categorizing and ranking images. Looking well beyond that ALT text on the images, Google is beginning to look at how the images are used on the site, who (and how) the images are being linked to, and even looking _within_ the images themselves to extract faces, text, and more.
